EDUCATION REGISTRATION FORM

Classroom or Zoom Classes (30 Hours)

• All students must be present on the first day of class per MVA policy. If a student misses Day 1, their enrollment will be canceled, and they must enroll in a future cycle.
• All students must begin classroom sessions (Zoom classes) with Unit 1.
• Zoom classes are virtual; students must join with a device equipped with a working camera and microphone.
• Attendance is mandatory for every session. Students must participate in all classes, complete all units, quizzes, and pass the final exam with at least 80%.
• The final exam can only be taken after completing the full course.
• If a student misses a class, they must make up the missed session (not just the time).
• Students who miss more than 12 hours (4 classes) must restart the course.

Behind-the-Wheel (BTW) Training (6 Hours)

• Students must pass the knowledge test and obtain a learner’s permit before starting BTW.
• All students must meet their instructor at the school location (no pick-up/drop-off services).
• If a student arrives without a valid permit/license, is impaired, or wears inappropriate footwear, the lesson will be canceled and marked as a no-show, incurring a $60 fee.
• If an instructor deems a student unprepared for the BTW phase, they may recommend additional practice before proceeding (with your coach or purchasing extra practice and get help from professional). If a student fails the final BTW session, they must arrange a retake and pay for 2 additional hours.
• Students must cancel or reschedule at least 48 hours in advance to avoid a $60 fee.
• Each 2-hour session includes a 10-minute break for feedback.
• Students are scheduled for three 2-hour BTW sessions and must contact the school (phone/text) to schedule them after completing Unit 3.

1. Students have the right to a certified instructor and must arrive on time and prepared, avoiding distractions (e.g., texting, talking in class).
2. Students are entitled to 30 hours of engaging classroom instruction and must actively participate.
3. Students will receive 6 hours of BTW training and must listen to the instructor and drive responsibly.
4. Students must be courteous and respectful to instructors and peers.
5. Classes will be held in a safe, clean, and well-equipped environment. Students must respect school property.
6. Students and parents have the right to visit the school and verify the instructor’s license and certification.
7.  student has the right to finish the education course (30 hours classroom + 6 hours BTW) within 18 weeks.

8. If student or parent have any complain or concern feel free to contact Sam driving school at 202-600-0889/ 301-726-3030. Each parent/driver coach and student has the right to place a complaint with MVA regarding problems associated with the driving school or the instructor at 410-424-3751. With that right each parent/driver coach and student has the responsibility to attempt to promptly pay the driving school for the driver education class and to attempt to alert the owner or manager of the driving school about any problems or complaints before contacting and outside agency.

Refund Policy (36 Hours – $420)

            Zoom classes cost:$30

BTW session cost: $130 per session.

• Refund deductions based on course progress:

Withdrawal after Zoom + 1 BTW session: $30 + $130.

• Withdrawal after Zoom + 2 BTW sessions: $30 + $260.
• Discounts/promotions do not waive these fees.
